Does the 231C form need to be signed by the landlord?

Does it need to be signed by the landlord? the form we are using from our attorney does not have a signature page. So would you suggest adding a signature page then?

That's why I provided the link to the one that the state is providing now. I would only use that form. It has a signature page at the end, and it does have a spot at the top where you list the name of the tenant and the landlord.
